Note: Your appointment confirmation email will have the appointment date, time, and office address. You will also receive an email with a link for your intake forms.
Our consultation fee is $179. This fee includes consultation, recommendation, and orders for up to seven months. We charge a $50 scheduling fee, the balance will be collected the day of your appointment.
Please keep in mind that in order to provide quality service, intake forms MUST be filled out at least 24 hours prior to your appointment. If the intake forms are not completely filled out you may be subject to a $50 fee and also be rescheduled, per company policy.

CANCELLATION POLICY
We aim to be as transparent as possible with our refund policy. It is as follows:
- Cancel up to 24 hours prior to your appointment — 100% refund
- Reschedule/no-show/cancel appointment less than 24 hours prior* – $50 fee
- *If the scheduled exam falls within 24-hours of booking, the refund could be subject to the $50 fee.
- Once a recommendation is issued after your exam, our services are non-refundable in any way.
