Schedule an appointment
Patients can schedule an appointment online or by phone at 954-240-6706. We will email you our new patient forms and ask that they be filled out before your appointment. Everything is done electronically and is HIPAA compliant.
Qualifying Medical Record
Patients should bring medical records to their appointment. Medical documentation that supports a qualification for a medical marijuana card can include, but is not limited to: a medical note, a prescription medication, or radiology report.
Patients must be either a permanent or seasonal resident of Florida to qualify for a medical marijuana card.

Medical Marijuana Consultation
Same day registration
After the consultation, patients must apply for their medical marijuana card through the Florida Medical Marijuana Use Registry (MMUR). Our office staff will do this for you with you to ensure it is done correctly. We will also review the various Medical Marijuana Treatment Centers (MMTCs), discounts available to new patients, and answer any remaining questions patients may have. Lastly, we will continue to follow up with you until your approval is complete.
Please note there is a $75 processing fee due (+ $2.75 convenience charge) to the State of Florida for your application.
Fill Your Recommendation
It takes about 10 business days for new patient applications to be processed. Upon approval, patients will be emailed a temporary card at which point they can begin purchasing product from any MMTC either by walk ins, place orders online for pick up, or by delivery!
A physical medical marijuana card typically arrives in two-three weeks after approval.
